Preventive Health Care

The Healthier SG programme is a multi-year national initiative that focuses on preventive health by empowering residents to proactively take steps in managing their health, to prevent or delay the onset of chronic diseases. 

Healthier SG aims to transform the way healthcare is delivered, by reshaping individuals’ health-seeking behaviours and lifestyle, thereby preventing them from falling ill. Currently targeted at Singaporeans aged 40 years and above, the outreach is focused and multi-pronged for wider penetration by age, ethnicity and interest groups. Anchored to One Family Clinic

To achieve this, Healthier SG anchors each resident to a family clinic which holistically manages his or her health, and is supported by an eco-system in the community comprising the Healthcare Cluster, the Primary Care Network (PCN) and community partners. The residents have access to a customised Health Plan which is regularly reviewed, with recommended screenings and vaccinations, and community activities to support them in their health journey. The programme also offers subsidies under the Healthier SG Chronic Tier to ensure affordability of selected common chronic medications in the Healthier SG Medication List.

Healthier SG Implementation Office

The Healthier SG Implementation Office (HSI) works closely with MOH, the healthcare Clusters, PCN, GPs and its other partners to drive the Healthier SG objectives and develop the key strategies to transform primary and community care. The project management includes the following:

  • Development of care protocols and integrated programmes, IT systems and applications, public communications, patient care and financing support
  • Engagement of GPs and Clusters to adopt the care protocols and programmes
  • Support efforts to boost public enrolment, community partnerships and social prescriptions, a key enabler for sustained interest and behavioural changes to improve health outcomes
  • Support for data and IT requirements to implement and monitor progress and outcomes
  • Secretariat and corporate services support for key Healthier SG meetings
  • Budget management of overall Healthier SG strategy to optimise delivery of outcomes
