Our Problem Statement:

“How can health and social care providers better work together to facilitate shared care for patients?”

While many hospital and community programmes are created to support patients in the community, these services are provided by different agencies, institutions and professionals. This can lead to:
• Limited visibility
• Duplicative services
• Fragmented repositories

Efficient information sharing is central to shared care. An application for patient data sharing could enable seamless collaboration and the delivery of integrated, holistic care.

Introducing the OCP Solution

Launched in October 2023, the OCP solution was developed in close partnership with a dedicated team of community nurses, social care providers, and medical social workers. It is a web-based application for data sharing across providers, enabling shared care in the community.

Through OCP, a multi-professional or multi-provider team can share one holistic view of a patient’s care needs and care plans.

We focus on patients with complex health and social needs, addressing the unique challenges they face in the community setting.

The OCP Application facilitates secure information sharing among providers, including community nurses, hospital care teams, and social care organisations.

With convenient access to patient data, providers can craft holistic care plans encompassing both healthcare and social care, ensuring patients receive comprehensive support.
